Qantas flew an Airbus A380 from Sydney to Dallas-Fort Worth and back with a 33 cm maintenance work light left inside the left wing after work in Sydney on 7 January. The object remained undetected through two long-haul sectors before being found when the aircraft returned to Sydney on 9 January, prompting the Australian Transport Safety Bureau to highlight breakdowns in tool accountability and foreign-object inspection procedures.
The aircraft involved was registered VH-OQK, and investigators found no damage or operational impact from the incident. Even so, the case underscored the risks posed by foreign objects in aircraft maintenance, particularly on widebody aircraft that undergo complex servicing checks before and after long-haul operations.
Qantas has since tightened tool-control procedures and introduced a mandatory pre-clearance check to confirm all tooling is accounted for after maintenance. The event was voluntarily reported and follows an earlier Qantas A380 tooling incident, keeping maintenance discipline and oversight under scrutiny at the airline.
